In order to make the program personal, please complete the questionnaire in its entirety and return it as soon as possible to drbrimer@eyecarewithfocus.com. If you utilize a paper format for your new patient intake forms, please include a PDF of those forms. If your headshot is not on your website, please attach one (it does not need to be a professional photo).
Also, we have put countless hours into preparing this great resource for you. However, it is especially for you, so we ask that you sign a Confidentiality Agreement, committing not to distribute the resources to others.
